Client age range:   12-18
Youth served :   approximately 75 per year
Referral source:  

Referrals are made by Sonoma County Juvenile Probation and Mental Health.
Sonoma County Mental Health clinicians and ACT Probation Officers assess candidates for possible inclusion in the ACT program.
If the youth and family qualify, the case is scheduled for ACT Court where specific orders to cooperate with ACT are mandated.

Eligibility requirements:  

To qualify, the youth must be on probation and meet the following criteria:

  • Medi-Cal eligible
  • The minor must be seriously emotionally disturbed and under 18 years of age at the time of entry
  • Have a DSM diagnosis other than:
      • Developmental disorder
      • Primary substance abuse disorder
      • Primary conduct disorder
      • No history of violence

In addition, the minor must have substantial impairment in two or more of the following:

  • Self care
  • School functioning
  • Family relationships
  • Ability to function in the community
  • Either at risk of home removal or has been removed from the home

Support across life domains as needed in the family/youth plan
24 hour, 7-day-a-week crisis response available
